Bug Report/Feature Request Form in Input

It would be great to add a bug report modal form to the Input platform. When a user submits a bug report or feature request, it creates an issue or ticket in the Input repository with the information provided by the user.

To combat spam, authentication, such as GitHub authentication, would be necessary before allowing users to submit their reports or requests.
